Who operates the website
The publisher and public contact route are displayed on this page and on About. Use Contact for a website-privacy question. Do not include patient records, wound photographs, diagnoses, insurance identifiers, or other sensitive health details in a request. This is not a patient portal or a service for managing medical records.
Information entered in forms
If you submit a contact message or practice listing, the website receives the fields you submit, such as name, email, public practice details, and message text. The information is stored in a private administrative inbox for handling the inquiry or verifying the proposed listing. Sending a form does not make its contents public automatically. Email delivery, when enabled, uses the configured mail service to handle messages.
If newsletter signup is available, the website stores the email address, selected interest, and subscription status. A confirmation process is used before newsletter delivery, and unsubscribe controls stop future queued newsletter delivery. The publication does not ask for a diagnosis to subscribe. A disabled signup feature should not be treated as an invitation to submit an address through another form.
Browser tools and saved reading
The appointment planner, care-call planner, and research notebook process their inputs in the browser tab. Their code does not submit the entries to this server, add them to a URL, or persist them in browser storage. Downloading, copying, or printing creates a copy under your control. Avoid entering sensitive medical information even in a local note.
The separate Save feature for reading resources stores selected resource identifiers in the browser's local storage. It is not an account-synchronized reading history. Other people using the same browser may be able to see those saved resources. You can remove saved items or clear the site's browser data.
Cookies, server logs, and security
The application uses a session cookie for functions such as sign-in and form security. It may use a preference cookie when you choose to block third-party advertising. Hosting and security systems may receive technical information such as request paths, timestamps, IP addresses, browser details, and error records. Optional application usage counters are controlled by the publisher and are not a medical-outcome dataset.
Administrative access is restricted, but no website can promise absolute security. Do not treat a contact form as a secure clinical record-transfer service. The hosting provider and configured service providers may process operational information necessary to run the site. Applicable provider retention and access practices must be assessed separately from the application.
Advertising and consent choices
Where advertising is enabled, Google and its partners may use cookies or other identifiers and process information to provide, measure, and protect advertising services. Google describes its practices in its Privacy Policy and advertising information. A label such as non-personalized does not mean no cookies or no data processing.
Where the Google consent message applies, use it to accept, decline, or manage relevant choices. The site's advertising controls also suppress its Google-ad integration when a supported block-ad preference or Global Privacy Control signal is detected. That does not remove earlier third-party cookies or change choices on another website. The disclosure pages and local worksheet tools do not load the site's advertising or consent scripts.
Keeping and removing information
The publisher uses submitted information for the purpose for which it was provided and for necessary security and operational records. The application supports removal of subscriber information and moderation of inquiries; protected rotating backups may retain previous snapshots until they expire. Hosting logs, exported copies, and third-party service records can follow separate retention processes. There is no claim that a request instantly erases every backup or third-party copy.
Contact the publisher to ask about access, correction, deletion, or advertising choices relevant to you. We may need enough information to locate the record and confirm that you are entitled to request a change. Do not send identity documents unless a specific, appropriate secure process has been arranged. Your applicable rights depend on the relevant law and circumstances.
Links and policy changes
Following an external link takes you to a service with its own practices. We do not control those sites. Material changes to how this website collects or uses information require updating this policy and the relevant settings. A policy is a description of actual operation, not a substitute for implementing the described controls.
